The challenges facing midwives

  • Antenatal care follows a calendar

One appointment a month, then every two weeks. The rhythm is known in advance and rarely booked in advance.

  • Birth preparation happens in groups

Eight sessions, six couples, sign-ups by email and a spreadsheet that overflows.

  • Home visits

Driving not counted, addresses sent by text, days that overrun.

  • Emergencies that shift everything

You leave, and the next three appointments have to be told one by one.

Practical guide: Midwives

  1. Book the antenatal schedule on recurrence from the first appointment: the rhythm is known.
  2. Preparation sessions declared with a number of places: sign-ups close by themselves.
  3. Travel charges by area and buffer time equal to the average drive for home visits.
  4. A required "address, floor, entry code" field for visits: ten minutes saved every time.
  5. Keep a daily buffer slot: in this work, the day moves.

Frequently asked questions

How do I manage home visits?

As a separate service, with travel charged by area and buffer time for the drive.

Does Kagenda hold clinical records?

No. It is a diary.

What applies to independent midwifery in the UK?

Registration first: "midwife" is a protected title, you must be on the Nursing and Midwifery Council register, and article 45 of the Nursing and Midwifery Order 2001 makes it a criminal offence for anyone other than a registered midwife or doctor to attend a woman in childbirth except in a sudden emergency or while training. Revalidation runs on a three-year cycle. The hard part of independent practice is indemnity: since 2014 it has been a legal requirement for registration to hold an appropriate indemnity arrangement covering your scope of practice, and the availability and cost of cover for intrapartum care has repeatedly constrained the sector — check what your cover actually includes before advertising birth services. Add CQC registration where the activity is a regulated activity.
